After you fill out this order request, we will contact you to go over details and availability of procedures to choose from. Model will only pay cost of consumables of the procedure they choose. Also models are being recorded for marketing or training staff purposes. Consents will be signed upon appointment confirmation.  There is a $50 dollar fee for appointments and fee is then applied towards model procedure cost.
